Completing Assessments and Using Rating Scales

Overview 

ICANotes+ supports standardized clinical assessments that can be completed either by staff within the application or by clients through the Client Portal.

Completed assessments automatically calculate the appropriate score. The assessment results and rating scales can be included in supported clinical notes, including:

  • Progress Notes
  • Case Management Notes
  • Complete Evaluations
  • SOAP Notes

Assessments can be completed using one of the following methods:

  • Record the assessment directly in ICANotes+.
  • Send the assessment to the client through the Client Portal.

Record an Assessment in ICANotes+

  1. Open the client's chart.
  2. Select the Chart Face tab.
  3. Click Forms / Assessments.

  1. From the Category list, select Clinical Rated Assessments.

  1. From the Form list, select the assessment to complete (for example, PHQ-A).

  1. Click Create.

  1. Complete the assessment.
  2. Click Submit.

A PDF of the completed assessment is generated.


Scroll to the scoring section of the PDF to view the automatically calculated assessment score.




Send an Assessment Through the Client Portal

  1. Open the client's chart.
  2. Select the Communications tab.
  3. Expand the Client Portal section.
  4. Click Select Forms and Assessments.

  1. Expand the Assessments section.
  2. Select the assessment to send (for example, PHQ-A).
  3. Click Send Form Request Email.

The assessment request is sent to the client and displays an Sent status.


The client receives a notification on the Dashboard in the Client Portal. The form can also be accessed from the My Documents tab in client portal.


The assessment appears with a Pending Review status in the My Documents tab until it is completed.


The client opens the assessment, answers each question, and selects Submit.


After submitting the assessment, the client receives a confirmation message.


The assessment status changes to Completed.



Add Assessment Rating Scales to Notes

Completed assessment scores can be added to supported clinical notes, regardless of whether the assessment was completed in ICANotes+ or through the Client Portal.

To view an assessment rating scale in a note:

  1. Open the clinical note (for example, a Progress Note).
  2. Select Rating Scales from the left panel.
  3. Select the completed assessment.
  4. Review the calculated assessment score displayed in the rating scale.

Can assessments be completed without using the Client Portal?
Yes. Assessments can be completed directly within ICANotes+ from the client's chart.

Can clients complete assessments remotely?
Yes. Assessments can be sent through the Client Portal, allowing clients to complete them remotely.

Where can I view the score for a completed assessment?
The calculated score is available in the generated PDF after the assessment is submitted. It can also be viewed from the Rating Scales section of supported clinical notes.
