Creating a Note from an Appointment

Overview 

When you schedule an appointment from the Calendar, you can create a note directly from that appointment. Creating the note at the time of scheduling automatically links it to the appointment, helping streamline your documentation workflow.

This article explains how to create a note while adding or editing an appointment and how to verify the link between the appointment and the note.


Assign Permission to Create a Note

Only authorized users can create a note from an appointment. To grant this permission, go to Settings > Practice Settings > Roles & Permissions, edit the appropriate role, expand the Notes section, and select the Create Notes checkbox.


Step 1: Add or Edit an Appointment

To begin, open the appointment from the Calendar.

  1. Navigate to Calendar.

  2. Do one of the following:

    • Click Add Appointment.

    • Select an existing appointment slot to open the Session Details slide-out panel.



Step 2: Access the Create Note Option

After opening the appointment screen, locate the option to create a note.

  • If you are adding a new appointment, the Add Appointment slide-out appears.

  • If you are editing an existing appointment, the Session Details slide-out appears. Click the Edit icon in the upper-right corner.

In the slide-out panel, locate the Create Note dropdown.



Step 3: Select the Note Type and Save

Next, choose the type of note you want to create and decide how you want to proceed.

  1. From the Create Note dropdown, select the appropriate note type. Available options may include:

    • Quick Note

    • Case Management

    • Couples/Family Therapy

    • Progress Note

    • SOAP Note

    • Complete Evaluation

    • Discharge Summary

    • Clinical Summary

    • ASAM

    • Clinical Message

    • Primary Care Note

  2. After selecting the note type, choose one of the following options:

    • Save and Open Note – Saves the appointment, plots it on the calendar, and opens the selected note so you can begin documenting.

    • Save – Saves the appointment and returns you to the Calendar without opening the note.


Step 4: Identify Appointments with Linked Notes on the Calendar

Once an appointment is saved with a note, you can easily recognize it from the Calendar view.

  • A note icon appears on the scheduled appointment.

  • Click the appointment to open the slide-out panel.

  • Select the linked note to open it directly.



Step 5: Verify the Linked Appointment

You can confirm the appointment link directly within the note.

  • At the top of the note, a message indicates that the note is linked to a scheduled appointment.

  • Click the appointment link in the message to open the appointment on the Calendar.



Step 6 (Optional): Change the Linked Appointment

 If necessary, you can link the note to a different appointment.

  1. Click Change Appointment within the note.

  2. A list of the client’s appointments appears.

  3. Select the appropriate appointment.

  4. Click Save to confirm the change.

Can I create a note for an existing appointment?
Yes. Open the appointment, click Edit, and select a note type from the Create Note dropdown.

What happens when I select Save and Open Note?
The appointment is saved to the calendar, and the selected note opens immediately for documentation.

How can I tell if an appointment has a linked note?
A note icon appears on the appointment in the Calendar.

Can I change the appointment linked to a note?
Yes. Open the note, select Change Appointment, choose a different appointment, and save your changes.
