Creating an Intake Package for Client Portal Users

Overview

You can send clinician-rated assessments to Client Portal users individually or as part of a package. Form & Assessment Packages allow you to group multiple forms together so they can be sent more efficiently through the Communications tab in the client chart.

This article explains how to create, update, manage, and delete Form & Assessment Packages.

Notes
You can also create a default package that is automatically sent to Client Portal users during sign-in, eliminating the need to send packages individually.

Create a Form & Assessment Package

  1. In the left sidebar, select Settings.
  2. Select the Practice Forms tab.
  3. In the left pane, select Form & Assessment Package.
  4. Click Create Package.



Add Forms to a Package

On the Form & Assessment Package Creation screen:

  1. Enter a title for the package.
  2. Expand Clinician Rated Assessments.
  3. Select one or more forms to include in the package.
  4. Click Save.

Update a Package

After creating a package, you can modify it at any time by adding or removing forms.

  1. Locate the package you want to update.
  2. Click the Edit (pencil) icon.
  3. Make the necessary changes.
  4. Click Save.




Create Multiple Packages

You can create multiple packages based on your practice needs. All created packages are available when sending forms to Client Portal users from the Communications tab.

  1. Navigate to the Form & Assessment Package section.
  2. Click Create Package.
  3. Configure the package as needed.
  4. Click Save.




Delete a Package

You can delete packages that are no longer needed.

  1. Locate the package you want to remove.
  2. Click the Delete icon.
  3. In the confirmation message, confirm the deletion.

FAQs 

Can I include multiple forms in a single package?
Yes. You can include one or more clinician-rated assessment forms in a single Form & Assessment Package.

Can I create more than one package?
Yes. You can create multiple packages based on your workflow or client needs. All available packages appear when sending forms through the Communications tab.

Can I edit a package after creating it?
Yes. You can update a package at any time to add or remove forms.

Can I delete a package?
Yes. Packages that are no longer needed can be deleted from the Form & Assessment Package section.

Will deleting a package delete the forms themselves?
No. Deleting a package removes only the package configuration. The individual forms remain available in the system.

Where can I send a package to a Client Portal user?
Packages can be sent from the client's chart through the Communications tab.

Can I create a default package for all Client Portal users?
Yes. You can configure a default package that is automatically sent to Client Portal users during sign-in.

Are changes to a package reflected immediately?
Yes. Once you save the changes, the updated package configuration is available for future use.
