Uploading a Form to the Client Portal

Overview

This article demonstrates how practice administrators can upload a fillable PDF form or an information sheet that can be sent to the client portal for download.

Step 1: Navigate to the Forms Section

  • Select Settings from the left sidebar.

  • Under the Practice Forms tab, navigate to the Forms section.


Step 2: Select the Upload Option

  • Select the Form Type as "Upload a form".

  • Click Add.


Step 3: Upload the Form

  • Acceptable file formats include: PDF, JPG, PNG, GIF, DOC, DOCX, XLS, XLSX, and TXT.

  • Each file may not exceed 20MB, and multiple files can be uploaded at a time.

  • Click Select Files to browse and select the file(s) you want to upload.

  • Double-click the selected file to open it automatically or click Open.


Step 4: Add Form Details

  • Enter a Title and Description for the form.

  • Click Upload to complete the process.


Step 5: Confirmation and Access

  • A notification will confirm the successful upload.

  • Click Close to exit the upload window.

  • The newly uploaded form will appear in your Forms list.

Is there a file size limit for uploads?
Yes. Each uploaded file must be 20 MB or smaller.

Can I upload multiple files at the same time?
Yes. Multiple files can be selected and uploaded during the same process.

Where can I access uploaded forms after uploading them?
Uploaded forms appear in the Forms list under the Practice Forms section.
