How to Link Note to Treatment Plan
Overview
This knowledge base article guides you to link notes to the treatment plan. The following notes can be linked to the treatment plan.
- Progress Note
- Case Management
- Complete Evaluation
- Couples/Family Therapy
Linking Progress Note to Treatment Plan
On the Progress Note, locate the Link to Treatment Plan button in PN, Part 2 tab. When you click this button and link treatment plan details, they will appear in the Instructions and Recommendations section as shown below:
Clicking the Link to Treatment Plan button opens the following screen. You have an option to choose from Geneal and Chemical Dependency Treatment Plan.
After you select the treatment plan, the following dropdown will appear with the list of all the problems recorded in the treatment plan chosen.
Click the following link to select the interventions.
In the following dropdown, select an applicable option for the progress in meeting goals and resolving problems.
In the following dropdown, select an applicable option for the ongoing plans for treatment.
Linking Case Management to Treatment Plan
On the Case Management Note, locate the Link to Treatment Plan button. When you click this button and link treatment plan details, they will appear in the Instructions and Recommendations section as shown below:
Note: The steps after clicking the Link to Treatment Plan button are the same as those explained in the Progress Note section above.
Linking Complete Evaluation to Treatment Plan
On the Complete Evaluation Note, locate the Link to Treatment Plan button. When you click this button and link treatment plan details, they will appear in the Instructions and Recommendations section as shown below:

Linking Couples/Family Therapy to Treatment Plan
On the Case Couples/Family Therapy Note, locate the Link to Treatment Plan button. When you click this button and link treatment plan details, they will appear in the Instructions and Recommendations section as shown below:
