
Navigate to the Chart Room.
Select the client from the client list.
Select the Documents tab.
Click Upload Documents.
The Attach Files window opens.
Click Select Files.
Browse to and select one or more files from your device.
Supported file types:
Multiple files can be uploaded at the same time.
Individual files cannot exceed 20 MB.
Supported image formats (.jpeg, .webp, and .avif) can be viewed directly in the Documents tab and the Client Portal. Image previews will display automatically where supported.
After selecting a file, enter the document details.
In the Attach To drop-down list, select one of the following:
Chart to attach the document to the client chart.
Episode of Care to attach the document to a specific episode.
In the Category dropdown, select the appropriate document category, such as Assessment Tools, Lab, Intake Forms, or Prescriptions.
In the Received field, the current date is selected by default. Update it, if needed, to reflect the actual date the document was received.
If you selected the wrong file, click the X next to the file name to remove it from the upload list.
To upload more files:
Click Add More.
Select additional files from your device.
Repeat the document detail steps as needed.
Click Upload.
A green check mark appears next to each file that uploads successfully.
To upload additional files, repeat the steps above.
Click Cancel to return to the client chart.

What file types can I upload?
The following file types are supported: PDF, JPG, JPEG, PNG, GIF, WEBP, AVIF, DOC, DOCX, XLS, XLSX, TXT, and HL7.
What is the maximum file size allowed?
Each individual file can be up to 20 MB in size.
Can I attach a document to a specific Episode of Care?
Yes. In the Attach To drop-down list, you can choose to attach the document to either the client chart or a specific Episode of Care.
Are manually uploaded documents integrated with rating scales?
No. Manually uploaded documents are stored for record-keeping purposes only and do not integrate with rating scales.
Can JPEG, WEBP, and AVIF image files be viewed after they are uploaded?
