How to Upload Portal Forms

ICANotes provides some forms that can be uploaded to the upload site for your patients to fill out. For information about all features including printable PDF instructions for your office and your patients, please see our Premium Patient Portal Knowledge Base Article.

In order to upload your forms, you first need to be marked as a Portal Portal Admin in the Security Center:
 
 
 

If you are a Patient Portal Admin, you should be able to go to the upload site at https://upload.icanotes.com and log in using your ICANotes user name and password.
 
  
 

Once you've logged in, you'll see several options on your left-hand side of the screen; the one we're looking for is Group Forms
 
 
 
In Group Forms, you have the option to select a folder (or 'Category') and view what forms are present within it.
 
 

In addition, you can choose your category on the right-hand side to choose which folder your new form goes into.
 
Once you've chosen a folder, you would click on  to find the form you'd like to upload on your local computer:
 
 
 
Please make sure your form is blank and does not contain any patient information in it!
 
This uploaded form will be visible to all patients on the patient portal.
 

Once we've selected the form and chosen  we can then name it and choose the folder it goes into.
 
 
 
And with this done, we'd choose  
 

This sends the form to the folder that you've specified!
 
 
 
From there, the form will appear in your patient's Patient Portal under Forms > Uploads/Downloads as a PDF document which they can download
