Sending Assessments / Intake Package to Client Portal

Overview

ICANotes+ enables clinicians to send assessments to portal-enabled clients through the Client Portal. Assessments can be sent individually or as part of an assessment package, allowing clients to complete required forms remotely before or after an appointment.

Clients receive an email notification when an assessment is available in the Client Portal.

Prerequisites

Before sending an assessment:

  1. Verify that the client has an active Client Portal account.
  2. Confirm that the client has a valid email address on file.
  3. Ensure the required assessment or assessment package is available in ICANotes+. (Refer to this knowledge base article - List of Clinician Rated Assessments in ICANotes+)

Send an Assessment Individually

  1. Open the client chart.
  2. Select the Communications tab.
  3. Expand the Client Portal section.
  4. Click Select Forms and Assessments.




  5. Expand the Clinician Rated Assessments section.
  6. Select one or more assessments to send.
  7. Click Send Form Request Email.

The client receives an email notification with instructions to complete the assessment through the Client Portal.




Send an Assessment Package

  1. Open the client chart.
  2. Select the Communications tab.
  3. Expand the Client Portal section.
  4. Click Select Forms and Assessments.




  5. Expand the Packages section.
  6. Select the assessment package to send.
  7. Click Send Form Request Email.

The selected package becomes available in the Client Portal for the client to complete.



Client Experience in the Portal

After the assessment is sent:

  1. The client receives an email notification.
  2. The client signs in to the Client Portal.
  3. The client opens the assigned assessment.
  4. The client completes the assessment questions.
  5. The client selects Complete or Submit.

The submitted assessment becomes available in the client chart for review.

Review Completed Assessments

  1. Open the client chart.
  2. Navigate to the completed assessment.
  3. Review the submitted responses.

Completed assessments are stored in the client record for future reference.








Troubleshooting

Client did not receive the assessment email
  • Verify that the client email address is correct.
  • Ask the client to check spam or junk folders.
  • Confirm that the Client Portal account is activated.
Assessment does not appear in the Client Portal
  • Verify that the assessment was successfully sent.
  • Confirm that the correct client received the assessment request.
  • Refresh the Client Portal session and sign in again.
Assessment remains in Pending Review status
  • Verify that the assessment was fully submitted by the client.
  • Refresh the client chart and reopen the assessment.

FAQs

Can clients complete assessments on a mobile device?
Yes. Clients can complete assessments using supported mobile devices, tablets, or desktop browsers.

Can multiple assessments be sent at the same time?
Yes. Multiple assessments can be sent together as part of an assessment package.

Can clinicians review completed assessments in the client chart?
Yes. Completed assessments are available in the client record for review.

Can an assessment be resent to a client?
Yes. The assessment can be sent again if the client did not complete the original request.
