Signatures Report

Overview

The Signatures Report helps clinicians and administrators track the progress of note signing and co-signing workflows. This report offers visibility into signed and co-signed documentation, including who signed, when signatures occurred, and whether co-signature requests have been fulfilled. By leveraging this report, practices can ensure timely completion of documentation, streamline co-signature follow-ups, and support compliance efforts.

This report can be exported in CSV, PDF, or Excel format, similar to other reports. Users must have the appropriate permission to access the Signatures Report.

Step 1: Accessing the Signatures Report

To access the Signatures Report:

  1. Select More from the left sidebar.
  2. Select Reports.
  3. In the Report dropdown, select Signatures Report.

Step 2: Using Report Filters 

By default, the report includes Start Date and End Date filters. The report filters results based on the encounter date

Additional filters are available to refine results:
  1. Signed: Select Y to view records with signed notes. Select N to view records with notes pending signature.
  2. Cosigned: Select Y to view records with co-signed notes. Select N to view records pending co-signature.
  3. Date Cosignature Requested: Select a date to view records where a co-signature was requested.
  4. Cosignature Request Sent To: Select the provider to whom the co-signature request was sent.
  5. Clinician: Select a clinician to view records for that clinician.
  6. Primary Insurance: Select a primary insurance to view records associated with the client’s primary insurance.
  7. Secondary Insurance: Select a secondary insurance to view records associated with the client’s secondary insurance.


Step 3: Running the Report 

After selecting the desired filter options, click the Run Report button. The report is then generated and displayed as shown in the screenshot below. A description of each column in the report is provided beneath the screenshot.



When you run or export the report, it includes the following columns:
  1. Encounter Date: Date when the service was rendered to the client.
  2. First Name / Last Name / Preferred Name: Client's identifying details.
  3. Chart ID: Client's chart ID.
  4. Medical Record Number: Client's unique medical record number (MRN).
  5. DOB: Date of birth of the client.
  6. Primary Insurance: Displays the client’s primary insurance.
  7. Secondary Insurance: Displays the client’s secondary insurance.
  8. Assigned Clinician: The clinician responsible for the note.
  9. Note Signed: Indicates whether the note has been signed.
  10. Note Co-Signed: Indicates whether the note has been co-signed.
  11. Co-Signature Request Sent To: Name of the provider who was sent the co-signature request.
  12. Date Co-Signature Requested: The date when the co-signature was requested.
  13. Date Signed: The date when the note was signed by the assigned clinician.
