List of All Assessments and Locations in ICANotes

ICANotes has an extensive list of public domain behavioral health assessment tools built into our program.
 
We have many custom forms and assessments available through the   button from an individual patient's Chart Face. To see a complete list of all of these custom forms, please see Custom Forms and Assessments.
 
AIMS (Abnormal Involuntary Movement Scale): This is available in the mental status examination area from the  button. 
 
 
ASAM is available in the Complete Assessment under the History Present Illness tab.
 
 
AIMS is available from the  button in the Nursing Note.
 
 
MMSE (Mini-Mental Status Examination): This is available in the mental status examination area in the Cognition/Dementia area. The  button will bring you directly to the assessment or you can go into the  then  button to enter the assessment. 
 
 
Tanner Scale: This is available under the  shrub in the mental status examination area.
 
 
Functional Assessment: Available from the Problem / Screens tab of the Pre-Admission is the  button. 
 
 
Medical Screen: Also available from the Problem / Screens tab of the Pre-Admission is the  button. 
 
 
Available from the Dimension 2 tab of the Chemical Dependency Assessment is the  button. 
 
 
CIWA-Ar: Available from the Dimension 1 tab of the Chemical Dependency Assessment is the  button. 
 
 
Global Assessment of Individual Needs: Available from the Dimension 3 tab of the Chemical Dependency Assessment is the  button. 
 
 
SNAP (Strengths, Needs, Abilities, Preferences for Treatment): From the Status tab of the General Treatment Plan, click the  button. 
 
 
SNAP is available from the Social History tab in a Complete Assessment. 
 
 
BPRS 18 (Brief Psychiatric Rating Scale): From the Status tab of the General Treatment Plan, click the  button. 
 
 
Fall Assessment: Available on the PN, Part 1 tab of the Prescriber Note, click the  button. 
 
 
Suicide/Violence Risk Assessment: Click the  button from the Therapist Note.
