Removing an Adverse Drug Reaction/Allergy from a Patient's Chart

If an allergy was put on a patient's chart but it does not apply, follow these steps to remove the allergy. 
 
Create a new note to access the Drug Reactions area, which you'll find in the following note types:
 
Complete Assessment (of Any Discipline)

Within the Complete Assessment, navigate to the Medical History tab and locate the combined ADRs and Allergies button. 
 



Proceed to List ADRs and Allergies.
 
 


Prescriber Progress Note

Within the Prescriber Progress Note, navigate to Drug Reactions in PN, Part 1. 
 
 


Nursing Progress Note

Within the Nursing Progress Note, locate the combined enter... Drug Reactions button. 


 

In the Adverse Drug Reaction (ADR) & Allergy List window, locate the ADR or allergy to be removed and click to select it. 
 
 



The ADR/Allergy information will populate in the work areas on the right. 

  
 


Set the Status from Active to Inactive, then set a Reason For Status Change by making a selection from the dropdown field (or you may free-text your own entry here). Click to Save your chnage(s). 
 
  



The ADR/Allergy listing on the left will update to reflect your changes.

 



Your note work area will update to reflect the change in status to the ADR/Allergy. 

 



Compile your note. The Chart Face and ADR/Allergy Log will update to reflect the changes made.
