Overview 

ICANotes+ enables you to document information within the ASAM note, which comprises six dimensions. After recording details for each dimension, you can generate and preview the note from the Recommendation tab. Within this tab, you can enter diagnosis codes, service codes, offer recommendations, document time spent, and insert the highest risk ratings. These entries will subsequently be incorporated into the compiled note.

Prerequisites 

To utilize the ASAM feature for your practice, it needs to be activated initially. Kindly reach out to our customer support team to activate this feature for your practice.

Set up ASAM Feature 

Once you've activated the ASAM feature for your practice, you can configure it through the Practice Settings and Personal Preferences sections, as illustrated in the screenshot below:

1. Navigate to Settings on the sidebar and access the Note Settings tab from the top.
2. Choose Note Settings from the left pane.
3. Check the box for the ASAM feature, as demonstrated on the screen.

To customize the ASAM feature using your personal preferences, follow these steps:

1. In the top right corner, click on the dropdown arrow next to your logged-in username.
2. Choose the Personal Preferences option.
3. Access the Note Settings tab from the top and then select Note Settings from the left pane.
4. Check the boxes for the ASAM feature, as displayed on the screen.

Document Information in ASAM 

After activating the ASAM feature for your practice and completing the required configuration with assistance from our customer support team, you'll find the ASAM option in the notes section on the Chart Face, as depicted below:

Upon selecting the ASAM option, you'll encounter the screen below, facilitating the recording of details across all six dimensions and the Recommendation tab.

ASAM DIMENSION 1: INTOXICATION / WITHDRAWAL 

In the Dimension 1 tab, you can find a table with the Risk Rating and Description. You can also record the shrub content for the following:

• Chemical Use History
• Withdrawal History
• CIWA: Alcohol Withdrawal History

Once you record the shrub content and select the desired risk rating, it appears in the yellow card on the right side as depicted in the screen below.  

Chemical Use History 

When you select the Chemical Use History button in the Dimension 1 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 1 tab with the risk rating details.

Withdrawal History 

When you select the Withdrawal History button in the Dimension 1 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 1 tab with the risk rating details.

CIWA: Alcohol Withdrawal History 

When you select the CIWA: Alcohol Withdrawal History button in the Dimension 1 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 1 tab with the risk rating details.

ASAM DIMENSION 2: BIOMEDICAL CONDITIONS / COMPLICATIONS 

In the Dimension 2 tab, you can find a table with the Risk Rating and Description. You can also record the shrub content for the following:

• Psychotropic Med History

• Current Non Psychotropics

• Medical Screen

Once you record the shrub content and select the desired risk rating, it appears in the yellow card on the right side as depicted in the screen below.   

Psychotropic Med History

When you select the Psychotropic History button in the Dimension 2 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 2 tab with the risk rating details.

Current Non Psychotropics 

When you select the Current Non Psychotropics button in the Dimension 2 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 2 tab with the risk rating details.

Medical Screen 

When you select the Medical Screen button in the Dimension 2 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 2 tab with the risk rating details.

ASAM DIMENSION 3: EMOTIONAL / BEHAVIORAL / COGNITIVE CONDITIONS AND COMPLICATIONS  

In the Dimension 3 tab, you can find a table with the Risk Rating and Description. You can also record the shrub content for the following:

• Global Appraisal of Individual Needs

• Mental Health Treatment History

• Mental Status Exam

• Immediate Risk Factors

Once you record the shrub content and select the desired risk rating, it appears in the yellow card on the right side as depicted in the screen below. 

Global Appraisal of Individual Needs  

When you select the Global Appraisal of Individual Needs button in the Dimension 3 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 3 tab with the risk rating details.

Mental Health Treatment History 

When you select the Mental Health Treatment History button in the Dimension 3 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 3 tab with the risk rating details.

Mental Status Exam 

When you select the Mental Status Exam button in the Dimension 3 tab, the following screen appears to enter the shrub content. 

Once you save the shrub content in the above screen, a separate section appears in the yellow card as highlighted in the screen below. You can also create custom content separately for the Mental Status Exam section.

Immediate Risk Factors 

When you select the Immediate Risk Factors button in the Dimension 3 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 3 tab with the risk rating details.

ASAM DIMENSION 4: TREATMENT ACCEPTANCE / RESISTANCE 

In the Dimension 4 tab, you can find a table with the Risk Rating and Description. You can also record the shrub content for the following:

• Dimension 4, Points to consider

Once you record the shrub content and select the desired risk rating, it appears in the yellow card on the right side as depicted in the screen below.

When you select the Dimension 4, Prior to consider button in the Dimension 4 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 4 tab with the risk rating details.

ASAM DIMENSION 5: RELAPSE POTENTIAL  

In the Dimension 5 tab, you can find a table with the Risk Rating and Description. You can also record the shrub content for the following:

• Relapse History

• CD Residential Treatment History

Once you record the shrub content and select the desired risk rating, it appears in the yellow card on the right side as depicted in the screen below. 

Relapse History 

When you select the Relapse History button in the Dimension 5 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 5 tab with the risk rating details.

CD Residential Treatment History 

When you select the CD Residential Treatment History button in the Dimension 5 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 5 tab with the risk rating details.

ASAM DIMENSION 6: RECOVERY ENVIRONMENT  

In the Dimension 6 tab, you can find a table with the Risk Rating and Description. You can also record the shrub content for the following:

• Support System/Social Setting

Once you record the shrub content and select the desired risk rating, it appears in the yellow card on the right side as depicted in the screen below. 

Support System/Social Setting 

When you select the Support System / Social Setting button in the Dimension 6 tab, the following screen appears to enter the shrub content. Once you save the shrub content, it appears in the yellow card within the Dimension 6 tab with the risk rating details.

Recommendation 

The Recommendation tab allows you to record diagnosis codes, service codes, time spent on session, select recommended level of care, and create custom content. Once you record these details, you can compile the note from this tab. 

Record Diagnosis Codes  

In the Recommendation tab, you can record the Diagnosis Codes as highlighted in the screen below. You can include the Diagnosis Codes in the notes, if needed.

Record Service Codes 

In the Recommendation tab, you can record the Service Codes as depicted in the screen below. You can include the Service Codes in the notes, if needed.

Make a Recommendation 

In the Recommendation tab, locate the Make a Recommendation button as highlighted in the screen below. Clicking this button opens a screen to record the shrub content. Once you record the shrub content, it appears in the yellow card on the right side.

Upon clicking the Make a Recommendation button opens the screen as shown below to record the shrub content.

Time Spent 

In the Recommendation tab, you can locate the Time Spent button that allows you to capture the time spent on the session as depicted in the screen below. 

Insert Highest Risk Rating(s)  

In the Dimension tabs, you'll find a table designated for recording risk ratings. This feature enables you to insert the highest recorded risk rating among all ratings, which will be showcased on the compiled note. If multiple risk ratings share the same highest value, all of them will be displayed on the compiled note. In the provided example, both Dimension 4 and Dimension 6 feature the highest risk ratings, consequently appearing on the Recommendation tab as illustrated below:

Create Custom Button  

In ASAM, you can create the custom content and include it on the compiled note. As shown in the screen below, when you click the Create Custom Button and create custom content, it appears in the Recommendation tab as highlighted in the screen. When you compile the note, the custom content appears on the note as well. 

When you click the Create Custom Button option, the Add Custom Content screen appears as shown below that allows you to create custom content. 

Compile and Preview ASAM Note 

In the Recommendation tab, locate the Compile And Preview button as highlighted in the screen below. Click this button to view the compiled note.

When you click the Compile And Preview button, the compiled note appears as shown below. The details captured in all the tabs will appear in the compiled note including custom content.

Capture Electronic Signature on the ASAM Note  

Upon compiling the ASAM note, you can capture electronical signature on the note. Locate the Electronically Sign button in the bottom right side on the compiled note.

When you click the Electronically Sign button, the captured signature appears on the compiled note as shown in the screen below. If needed, you can request the compiled ASAM note for co-signature to other provider by clicking the Request Co-signatures link as highlighted in the screen.
