How to Add Client Demographics

Overview

This knowledge base article guides you on adding client's demographic details in the system.

Adding Client's Demographics

Provide general and expanded demographic data and client contacts within the Demographics tab of the Chart Face. Each section (General, Demographics, Contact) can be expanded and collapsed as needed. 

 

 

 Client information entered at the time of chart creation will populate in their respective fields inside the General section. Click the pencil icon   to make changes. 

 

 

 

In the Edit general demographics pop-up, use available fields to enter additional details, including the client's Middle, Maiden, and Other Names, Sex, and Social Security number. 

 

 

 

Click Save or Cancel to return to the chart. 

 

Follow the same workflow of clicking on the pencil icon  to make modifications to Demographics

 

  

 

 

Click Save or Cancel to return to the chart. 

 

 

Provide the client's contact information, including email address (which will be required for Portal registration), phone numbers, and addresses, in the Contact section. You can hide the General and Demographics sections by clicking on their headers to collapse each container. Click again to expand each section.

 

 

 

Click to expand the Contact section. You may want to also click to collapse the General section. 

 

To add an email address, click the  plus icon 

 

 

 

When finished, click Close to save your changes.

 

To add a phone number, click the  plus icon 

 

 

Use the dropdown field to set the Type, then enter the Phone and Ext(ension). Then click the  plus icon.

 

 

As with email addresses, you can add multiple phone numbers as needed, with options to specify whether a phone number is bad/preferred. Click the trash icon  to delete a phone number, or the pencil icon  to make changes. 

 

 

When finished, click Close to save your changes.

 

To add an address, click the  plus icon 

 

 

Enter the Street Address, City, State and Zip code. If necessary, check the Bad Address checkbox. Then click the  plus icon.

 

 

As with emails and phone numbers, multiple addresses can be added before saving changes to the chart, with options to specify whether and address is bad/preferred. Click the trash icon  to delete an address, or the pencil icon  to make changes. 

 

 

When finished, click Close to save your changes.

 

If the client is homeless, you can indicate such with the Patient is Homeless toggle located at the top right corner of the Contact section.
