How to Add Client Demographics

Overview

This knowledge base article guides you on adding client's demographic details in the system.

When you access the client's chart, the demographics section is divided into four sections:
  1. General
  2. Demographics
  3. Contact
  4. Custom Fields

Adding Client's General Details

The General tab allows you to enter the following details:
  1. Legal First Name
  2. Legal Middle Name
  3. Legal Last Name
  4. Preferred Name (When the preferred name is different than the legal first name, it appears in parenthesis prior to the client's name in chart face ribbon.)
  5. Date of Birth
  6. Legal Sex
  7. Social Security#

 


These details are saved when you enter client's chart initially. To make any changes to these details, click the Edit button. Once you modify the details, click Save.




Adding Client's Demographic Details

The Demographics tab allows you to enter the following details:
  1. Gender
  2. Sexual Orientation
  3. Race 1
  4. Race 2
  5. Ethnicity 1
  6. Ethnicity 2
  7. Religion
  8. Nationality
  9. Employment status
  10. Income range
  11. Preferred language
  12. Education Level
  13. Marital status
  14. Family size
  15. Disability
  16. Veteran status

 


These details are saved when you enter client's chart initially. To make any changes to these details, click the Edit button. Once you modify the details, click Save.


  

 

Adding Client's Contact Details

In the Contact tab, you can record the following details:

  1. Email Addresses
  2. Phone Numbers
  3. Mailing Addresses

 


Add Email Addresses

In the Emails box, click the Edit button to change the existing email address or add a new one.

To add a new email address:
  1. In the Type dropdown, select the desired type. The options are: Personal, Work, Related Contact.
  2. Enter email address.
  3. Click Add.
  4. Click Close.
To change the existing email address, click the Edit button.
To delete the email address, click the Delete button.



Add Phone Numbers

In the Emails box, click the Edit button to change the existing email address or add a new one.

To add a new phone number:
  1. In the Type dropdown, select the desired type. The options are: Home, Mobile, Work, Related Contact.
  2. Enter phone number.
  3. Enter extension, if applicable.
  4. Click Add.
  5. Click Close.
To change the existing phone number, click the Edit button.
To delete the phone number, click the Delete button.



Add Mailing Addresses

In the Addresses box, click the Edit button to change the existing mailing address or add a new one.

To add a new mailing address:
  1. Enter street address1.
  2. Enter street address2.
  3. Enter city.
  4. Select state from the dropdown.
  5. Enter the zip code.
  6. Click Add.
  7. Click Close.
To change the existing mailing address, click the Edit button.
To delete the mailing address, click the Delete button.


 


If the client is homeless, select the Client is Homeless checkbox at the extreme right in the Contact tab.
