Creating a New Chart
Click on the button in the Chart Room.
A dialog box will open:
Enter the patient’s First Name, Last Name, and Date of Birth, then click the button.
The program will check to see if there is already a chart with that name and DOB or a similar name with the same DOB and alert you if that is the case to help prevent duplicate charts.
If everything is in order, the Demographics screen (see screenshot below) will now open and you will be on the Patient Information tab with the patient’s name and date of birth already populated. You may now enter the patient’s demographics on the Patient Information, Insurance Information, and Other Contacts tabs or you can click over to last tab, Telephone Intake, to enter all of the information on one screen.
Fields required to create a chart (before you can save the chart) are highlighted in red. After you populate these fields, you can create notes or appointments for the patient. Additionally, if you use, or plan on eventually using e-billing, you have the option to using the button at the bottom of the screen.
To enter for a patient, switch over to that tab. To create an insurance company for this patient, press
. Then click on the field labeled Insurance Co.:<Enter Carrier> and select the insurance company name from either the Full List or from your Short List.
Once you’ve entered the information you want or need to, you can proceed.
If you are making an appointment for this patient, click on the button at the top of the screen to go to the patient’s Chart Face, then click on the
button (left hand side) to go to the calendar to schedule an appointment for this patient.
Otherwise, if you want to start creating notes for this patient, you may choose one of three buttons located to the bottom-right of the demographics page to move forward:
Please note that you must assign at least one provider to your chart before you can exit the demographics; this pop up will alert you to the fact, and give you the option to quickly assign yourself as the provider.
