Configuring and Using the ICD-10 Shortlist

Overview

You can configure and record ICD-10 diagnosis codes in multiple ways while creating notes. When adding a diagnosis code, you can also assign the appropriate status. You can record diagnosis codes in the following ways:
  1. Narrow down from the alphabetical list.
  1. Type the ICD-10 code or description and select it from the populated list.
  1. Use the ICD-10 Shortlist tab, which displays only the configured ICD-10 codes.
This article explains how to configure the ICD-10 shortlist and use it when recording codes.

Step 1: Configure the ICD-10 Shortlist

You can configure the shortlist in two ways:

  • By narrowing down from the alphabetical list.

  • By typing a short description and selecting from the populated list.


  1. Navigate to Settings > Practice Settings > Diagnosis Short List.



  2. Click Add to List.

    The following screen opens:

  3. Configure diagnosis codes in three steps:

    1. STEP 1 - MAJOR GROUP CATEGORY: Select a major group category from the dropdown, for example: A00–B99, C01–D49, D50–D89.

    2. STEP 2 - NARROW SCOPE: Narrow the scope by selecting a subcategory from the dropdown.

    3. STEP 3 - SELECT DIAGNOSIS: Select the diagnosis code from the narrowed list.

  4. Click Add.

Alternate method:

  • In the Short Description field, start typing the code or description.


  • Select the ICD-10 code from the populated list.

  • Click Add.


Deleting Diagnosis Codes

You can remove diagnosis codes from your short code list. To delete a code, click the Delete icon on the far right side of the code entry.



 


Step 2: Use the ICD-10 Shortlist

  1. Open a client’s chart.

  2. Create a note and go to the Diagnosis page.

  3. Select the ICD-10 Shortlist tab.




    • This tab displays all the codes you configured in your shortlist.

    • Select the diagnosis code from the dropdown.


  4. After selecting the diagnosis code, select status from the adjacent dropdown. The following statuses can be assigned:

    1. Active
    2. Resolved
    3. Chronic
    4. Partial Remission
    5. Full Remission
    6. Maintenance Therapy, In Remission
    7. Relapse
    8. Inactive
    9. Recurrence
    10. Provisional



      In the following areas of the application, only ActiveChronicPartial RemissionRelapseRecurrence, and Provisional statuses will appear. Codes with other statuses will be excluded:

      1. Compiled Note
      2. Chart Face
      3. Carry Forwarded to Next Note
      4. Claims (Unbilled Encounters tab)
      5. Client Statements
      6. Labs Requisitions
      7. Superbill

      In the following areas of the application, all the statuses will appear.

      1. Treatment Plan
      2. Discharge Summary
      3. Clinical Summary
  5. Click Done.


Step 3 (Optional): Use Additional ICD-10 Recording Options

You can also record ICD-10 codes without using the shortlist.

  1. Open a client’s chart.

  2. Create a note and go to the Diagnosis page.

  3. Select the ICD-10 tab.

    • Narrow down your search using the alphabetical list.

    • Select status from the adjacent dropdown.

    • Click Done.


  4. Select the ICD-10 by Code tab.

    • Type the appropriate code or description.

    • Select the correct diagnosis code from the populated list.

    • Select status from the adjacent dropdown.

    • Click Done.
