How to Document an Episode of Care

The Chart Face section displays information relating to the client’s current episode of care. 




Each created chart contains a new episode of care. Click the pencil icon  to the right of the screen to make changes to the current episode of care. The Edit Episode of Care window will appear on top of the chart face. 

 

 

(1) Title - The Title will display at the top left of each episode of care within the chart

(2) Onset Date - By default, the Onset Date is the date the chart was created. Type or use the calendar dropdown to change the date.

(3) Discharge Date - Enter a Discharge Date to close the episode of care on the chart.

(4) Level of Care - Click the dropdown field to make an appropriate selection.

(5) Programs - If our practice administrator has created Programs in Program Settings, they will be listed in the Programs dropdown. Click to make a selection.

(6) Related - If applicable, check all related options for the episode of care

(7) Previously Treated - If the client has previously received treatment for care, click the checkbox and enter the Onset and Discharge Dates accordingly.

 

Click Update to save changes and return to the chart face. 

 

Should you need to re-open a closed episode of care, you may do so by clicking the pencil icon
